We will submit a claim to your insurance company.
However, your insurance company determines the amount of your coverage
and you
should be aware of your benefits. If you
have questions regarding your insurance coverage, you should contact
your
insurance representative. Payments for
all non-covered portions of the visit, including co-pays are payable at
the
time of service. Payments may be made by cash, check or credit
card. If we do not participate with your insurance plan, we ask
that you pay in full at the time of service.
